Our social service team assists patients and families during transitional periods, such as preparing for discharge. We also:

• provide essential information
• manage requests and concerns
• arrange for outside medical services such as dental, vision, audiology, and podiatry.

We can also assist in arranging for:

• psychiatric evaluations
• dementia assessments
• individual therapy

Other services include:

• legal assistance
• transportation to outside appointments
• accompanying residents (upon request) to those appointments.
