    24-Hour Skilled Nursing

    A stay at Linley Park Post Acute begins with an evaluation by our team of physicians, therapists, nurses, social services, and other support staff. These meetings allow us to design a personalized care plan focused on returning you to your highest level of independence as quickly as possible. Our team is available 24/7 to help you receive the highest level of care during your stay with us. Our goal is to help you be safe, healthy, and happy.  

    Complex Wound & Tube Management

    Our wound care program utilizes professional, clinical and technical resources designed for the treatment of a variety of wound conditions including:

    • Ostomies (colostomy/urostomy)
    • Wound dehiscence
    • Diabetic arterial/venous ulcers
    • Pressure ulcers
    • Pre- and post-flap surgery

    Diabetes Management

    Our experienced nurses provide help with all aspects of managing diabetes including:

    • blood glucose monitoring
    • insulin administration and tracking
    • making an exercise plan
    • menu planning

    Restorative Nursing

    At Linley Park Post Acute we seek to maximize the quality of life for our long-term care population who require a specialized restorative program. The therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:

    • Ambulation and range of motion
    • Activities of daily living
    • Bowel and bladder training
    • Strengthening and restraint reduction
    • Weekly and monthly height and weight monitoring

    Pain Management

    We understand that pain can delay rehabilitation progress.

    We use medication and other pain management techniques to make our patients as comfortable as possible so they can participate in and benefit from therapy.

    We also provide education and resources that patients will need to successfully manage pain when they return home.
